Final Descriptive Report - LIVABILITY:

The Final Descriptive Report - Livability (FDR - Livability) is due 90 days after the end of the award period. It consists of three Parts. All Parts must be submitted for the report to be complete. 

The FDR - Livability PDF form with Parts I & II must be e-mailed to FinalReports@arts.gov.  No scans will be accepted. Save a copy of the FDR - Livability PDF to your computer. The name of the PDF file should be FDR followed by the grant number (e.g., FDR 11-4292-7### or
FDR 12-4292-7###). Once completed, e-mail the FDR - Livability PDF to FinalReports@arts.gov with FDR, the grant number, and your organization's name in the subject line, e.g. FDR 11-4292-7### Arts Organization or FDR 12-4292-7### Arts Organization.
