Final Descriptive Report - STANDARD

For projects with the outcome of Creation, Engagement, Learning, or Understanding (see your award letter or My Grant at a Glance).

The Final Descriptive Report - Standard (FDR - Standard) is due 90 days after the end of the award period. It consists of three Parts. All three Parts must be submitted for the report to be complete. 

The FDR - Standard PDF form with Parts I & II must be e-mailed to A separate text field is allotted for each of the five questions in Part I. You do not need to use all of the space provided.

No scans will be accepted.  Save a copy of the FDR - Standard PDF to your computer. The name of the PDF file should be FDR followed by the award number (e.g., FDR 12-xxxx-xxxx.pdf). Once completed, e-mail the FDR - Standard PDF to with FDR, the grant number, and your organization's name in the subject line (e.g. FDR 12-xxxx-xxxx Art Organization).

Use Adobe Reader 9 or later. Mac user – do not use Preview, use only Adobe Reader 9 or later. If you don't have Adobe Reader 9 or above click Adobe Reader download.
