Final Reporting

Carefully review the How to Prepare Your Final Reports guide before completing your reports.  There are two versions, based on the fiscal year of the award (the last two digits of your award number):

Final Report Forms

Your final reports will be submitted entirely through REACH, except for any required hardcopy work product(s). Log in to REACH and open the Forms and Reports tab to view and submit the required reports for your award. Not all reports listed below will be required for your grant.

• Payment Request (only if you need to do a final draw down):

• Local Arts Agency Subgrants Report for ARP.
  For organizations that received an American Rescue Plan (ARP) 
  Grants to Local Arts Agencies for Subgranting.
  – Complete spreadsheet and then upload to the Forms and Reports 
     tab in REACH. 
  – LAA Subgrant Report for ARP: General Instructions and Spreadsheet
• Pass-through Entities Subawards Report (previously Local Arts Agency Subgrant Report).
  This report is used only by  awardees with approved NEA subgranting projects.  Check the Forms and Reports tab to see if you are required to submit this report.
  – Complete spreadsheet and then upload to the Forms and Reports 
     tab in REACH. 
  – Pass-Through Entities Subawards Report Instructions | Form

• Product Requirement.

  Research Awards
  – Research Labs:  Final Work Product Requirement for FY22
