NEA Jazz Masters Nomination Form

To make a nomination, enter the first and last name of the individual and indicate if the nomination is for the performer or advocate category. To complete the nomination, you must attach the two required PDF supporting documents as described in the guidelines, and then click submit.

A nominee must be a living U.S. Citizen or permanent resident of the United States.

Nominations must be submitted in English.

Nominator information

In the event of a major emergency (e.g., a hurricane or NEA systems technological failure), the NEA Chair may adjust application deadlines for affected nominations. If a deadline is extended for any reason, an announcement will be posted on our website.
