NEA Jazz Masters Nomination Form

To make a nomination, enter the first and last name of the individual and indicate if the nomination is for the performer or advocate category. To complete the nomination, you must:

Provide all the required information in the form below;
Attach the two (2) required PDF supporting documents as described in the guidelines; and
Click submit.

A nominee must be a living U.S. Citizen or permanent resident of the United States.

Nominations must be submitted in English.

The deadline is October 31, 2024.
