Final Descriptive Report - LIVABILITY

For awards issued before September 30, 2017.

For projects with the outcome of Livability only, including Our Town grants (see your award letter).

The Final Descriptive Report - LIVABILITY (FDR - Livability) is due 90 days after the end of the award period. It consists of three Parts. All three Parts must be submitted for the report to be complete.

The FDR - Livability PDF form with Parts I & II must be e-mailed to FinalReports@arts.gov. A separate text field is allotted for each of the five questions in Part I. You do not need to use all of the space provided.

No scans will be accepted. Save a copy of the FDR - Livability PDF to your computer. The name of the PDF file should be FDR followed by the award number (e.g., FDR 12-xxxx-xxxx.pdf). Once completed, e-mail the FDR - Livability PDF to FinalReports@arts.gov with FDR, the grant number, and your organization's name in the subject line (e.g. FDR 12-xxxx-xxxx Art Organization).
